This coronal (frontal view) T1 weighted MRI of the brain demonstrates a large, old stroke in the left (on your right) cerebral hemisphere, in the middle cerebral artery vascular territory. This region demonstrates encephalomalacia (atrophy) and is the dark region on the right side of the image. There is associated enlargement of the left lateral ventricle consistent with tissue loss. The left side (on your right) of the brainstem is smaller, reflecting Wallerian degeneration, which is downstream atrophy due to neuronal atrophy/destruction above the brainstem.
